56-year male presented with swelling in both groins with pain for 2-3 months. Initially, the swelling started on the right side almost 2 years ago and he noticed a bulge on the left just 6 months ago. Both these lumps were painless. He was tolerating mild discomfort for the last 4-5 months. The discomfort was more often and now, he developed pain for 2-3 months. Both the lumps appeared on standing and disappeared on lying down. He was evaluated and clinically, diagnosed with bilateral inguinal hernia. The same was confirmed with an ultrasound examination (sonography).
He was explained options of open, laparoscopic, and robotic surgery – advantages and disadvantages of all three surgeries discussed in detail. He decided for Laparoscopic bilateral inguinal hernia surgery. All necessary preoperative blood investigations, X-ray Chest and ECG were done, fitness for surgery taken.
The gentleman was admitted three hours prior to surgery with a six-hour fasting period. An antibiotic was given thirty minutes prior to surgery after the test dose. Under general anaesthesia, laparoscopic surgery performed through three small cuts – one cm under the navel and 0.5 cm on either side at the level of the navel. Using the navel cut for the telescope for vision, the surgery was performed using laparoscopic instruments on either side. The hernia was reduced, adequate space created and mesh placed on either side. After checking for any bleeding, the instruments were removed and all the wounds were closed with absorbable stitches and waterproof dressing.
The gentleman recovered well and was discharged the next day.
